New Scout and Parents!!!

Do you have the proper Medical Form for Summer Camp?

 

Summer Camp requires a Class 2 Medical Evaluation form, filled out by a licensed health care practitioner. The Class 2 Medical Evaluation is on the second page of BSA form #34414B. Click here for a copy of the form.

 

Older Scouts and Parents!!!

Are your Medical Forms up to date?

 

Class 2 Medical evaluations are required once every 36 months. Click the link above for a copy of the form if the one on file with the Troop is over 36 months old.

If your child has had a medical evaluation (physical examination) within the last 36 months, a copy of the results of this examination must be attached to the existing Class 2 Medical Form, provided it was filled out within the past 36 months.

 

Parents are required to re-sign the Class 2 Medical evaluations each year at the June Parent's meeting.

The "Citta Scout Reservation Prescription Medication Information"

and the "Citta Scout Reservation Over the Counter Medication" forms

must be filled out by all parent.

 

Both of these forms must be filled out and signed by all parents of scouts attending summer camp to acknowledge that

 

1. They are taking prescription medication while at camp.

2. They are not taking prescription medication while at camp.

3. The Health Officer may administer the Over the Counter medication listed on the OTC form.

4. The Health Officer may not administer the Over the Counter medication listed on the OTC form.
